Initial Phone or In-Office Consultation
(30 Minutes)

Full Assessment

This extensive evaluation and recommendation of individual and family needs takes place in your home or office. This comprehensive assessment evaluates our Client’s mental, physical and financial needs and makes recommendations regarding specific needs and/or identified services. A written report based on the assessment will be delivered within 5 business days of this assessment. Services by Golden Age @ Home are provided only when you determine they are necessary – This “Ready When You Are”. Assessment is valid for one year. Travel time & mileage are included within 30 miles of Tallahassee, Florida.

Care Navigation Services (monthly retainer)

Provides in-home visits and consultations regarding care needs and living conditions, overall care planning and plan oversight, care team coordination, and health care advocacy. This service requires that a full assessment be conducted prior to initiating care management services. Monthly care navigation services with an initial six (6) individual/ten (10) couple hour retainer. Additional hours are billed at the reduced rate of $75.00/hour under this monthly service retainer. Travel time & mileage are not included – please review terms regarding travel. Please note that unused hours do not roll over into subsequent month.

Care Navigation Services (per diem)

Provides in-home visits and consultations regarding care needs and living conditions, overall care planning and plan oversight, care team coordination, and health care advocacy. Brief Assessment

This short assessment allows us to understand the basics about our client including their living arrangements, special needs, and preferences. This service is included in the Decision-Making Plan and is required for both Personal and Telephone Check-in services. Assessment and coordination of health record portability can be completed electronically if necessary. Release of Information prior to communication with care givers will be required. Travel time & mileage are included – please review terms regarding travel.

Personal Check-Ins

The purpose of these visits is to assess the need for intervention or care management when family and/or caregivers are unable to determine needs independently. Ideal for clients who are traveling through the Golden Age @ home service area, away from home. Duration of visit – up to thirty (30) minutes. This service requires our one-time Brief Assessment prior to scheduling personal check-ins. This is so we have knowledge of our Client’s needs prior to visiting. Travel time & mileage are not included – please review terms regarding travel.

Telephone Check-Ins

The purpose of these calls is to assess the need for intervention or care management when family and/or caregivers are unable to determine needs independently. Ideal for clients who are traveling away from home. Duration of call – up to thirty (30) minutes. This service requires our one-time Brief Assessment prior to scheduling personal check-ins. This is so we have knowledge of our Client’s needs prior to visiting. This service requires our one-time Brief Assessment prior to scheduling telephone check-ins. This is so we have knowledge of our Client’s needs prior to calling.

Decision-Making Plan

This service helps plan for the responsibilities of Caregivers, Power of Attorney (POA), Health Care Representative, Trustee, or Personal Representative. It includes document organization and review; consultation and coordination with other involved professionals and Life Mapping. Travel time & mileage are included – please review terms regarding travel.

Counseling

One (1) hour counseling session for individual or family. Counseling creates a safe and confidential place for you to seek guidance from a trusted advisor, reflect on your life, and make changes.

Note: Video check-ins through HIPAA secured portals coming soon – available local connections will be required and additional charges may apply.

Additional Information

• Rates are based on regular operating hours: Monday – Friday, 8:00am – 5:00pm. Services provided outside of regular operating hours, including weekends and holidays, are at standard rate + 25%

• All hourly rates are charged in increments of fifteen (15) minutes after the initial hour.

• Travel time & mileage included in a service is restricted to within 30 miles of Tallahassee Florida. Additional travel time & mileage may apply for other locations.

• Mileage is charged at current Federal rate at the time of service.

• Golden Age @ Home does not accept insurance. Visa, MasterCard, American Express, personal check and cash are accepted.

• Returned checks subject to a $35.00 processing fee.
